Application for Child Care Services

The application for child cares services is now available in Adobe Acrobat .pdf format.  This application may be submitted online by completing the form fields within the .pdf file and using the submit button.  You may also choose to print and then mail/fax the completed application. You are not required to attach/send additional information requested at completion. You will be informed of what material is needed once your application is processed.
